Q Hello, I am 35 yrs old single male. Diabetic for last 10 years with poorly controlled glucose levels. Height: 168 cms, Wt: 87 Kg. HbA1c (on 3 Feb, 2013): 13.1 %. Avg BS: 329 mgs/dl. Then I have consulted endocrinologist and nephrologist, since Jan 26, 2013. Current Medication- for last 30 days-- Lantus 36 units (10 pm) Actrapid (22-22-22 units) Thyrox 125 mg od Ramipril 2.5 od Sitagliptin 50/500 bd Nefrosave od Rosuvastatin 10 od Kondro Od (Glucosamine)- for my knee problem, medication started 11 days ago**. Yesterdays biochemistry results: a. Serum Creatinine: 0.86 mg/dl. b. Serum K- 4.3 mmol/L b. fbs: 198, ppbs: 235 c. Urine Routine: Protein/Albumin- 1 +, Glucose- Trace d. Urine Microalbumin: 26.6 mg/L e. Urine Microalbumin/Creatinine Ratio: 35.4 f. Urine Sp Gravity: 1.03 g. Serum TSH: 0.419 micIU/ml h. FASTING INSULIN- 25.9 mIU/ml i. Serum C-Peptide (CLIA)- 1.32 ng/ml USG KUB - 10 MAR 2013: NORMAL RENAL AND URINARY BLADDER MORPHOLOGY. SMBG Results for last 26 days: Simple Avg-- i. FASTING: 219 ..range: 181- 280. ii. Before BREAKFAST: 238; range: 215-247 iii. PRE LUNCH: 239; range: 204-257 iv. POST LUNCH: 250; range: 187-330 v. PRE DINNER: 231; range: 144-313 vi. POST DINNER: 264; range: 219-361. Questions: 1. Am I type 1 diabetic now? 2. Despite taking Ramipril 2.5 od, Nefrosave od and diet restriction, Urine Microalbumin: 26.6 mg/L (last month it was 24.9 mg/L before starting these two drugs). Type 1 diabetic is found in young age. You have type 2 diabetes but under control. Urine microalbumin under 30mg is considered as normal. So what values you have is under normal range. All you need to do is take your medicine regularly and see to it that you follow strict diabetic diet and exercise regularly to prevent complications of diabetes.
